  • Workers' Compensation

  • AETNA

  • Cigna

Rates

Our rates vary, depending on the length of appointment, the training of the therapist, and the service provided. Please contact us for more information. Our therapist training levels include:

  • PhD Level Therapist

  • Master's Level Therapist (LPC/LCSW)

  • LPC-IT

  • Master's Level Intern

Insurance Information

Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

  • Do I have mental health insurance benefits?

  • What are my out-of-network benefits?

  • What is my deductible and has it been met?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session? Will I owe a copay or coinsurance for my sessions? 

  • Is approval required from my primary care physician?

Reduced Fee Services

Reduced fee services are available on a limited basis. Please call for more information.

Payments

Cash, check and major credit cards accepted for payment.

Copays, co-insurance, and deductible payments are due at time of service. You are also responsible for any portion of session fees not covered by your insurance company; and these payments are due at the time QUEST receives notification from your insurance company. QUEST does not determine copay or deductible amounts. You should contact your insurance company to understand your coverage.

A $10.00 service charge will be charged for any check returned for any reason.

Cancellation Policy

Please remember to cancel or reschedule 24 hours in advance of an appointment. If you do not cancel or reschedule 24 hours in advance of an appointment or fail to show for an appointment, then you will be responsible for the entire session fee (e.g., $215 per hour) which includes both the insurance portion and client portion related to a session. This is because QUEST cannot bill insurance for missed sessions.  

If you have out-of-network benefits or Worker's Compensation

Clients that would like to bill their session costs to their insurance for which QUEST is out-of-network, or to a Worker's Compensation program, pay for sessions upfront. QUEST will provide a superbill that you can give to your insurance company or Worker's Compensation for reimbursement.
